Answer: A claim is a request for payment that your healthcare provider sends to your health plan after you receive medical services. It shows what services were provided and what is being billed.
Answer: A copay is a fixed dollar amount you pay at the time of a medical service—for example, a $20 copay for a doctor visit. It usually does not count toward your deductible.
Answer: Coinsurance is the percentage of costs you pay after meeting your deductible. For example, if your coinsurance is 20%, you pay 20% of the covered cost and your plan pays 80%.
Answer: Your deductible is the amount you pay out-of-pocket for covered services before your health plan begins sharing costs.
Answer: The allowed amount is the maximum cost your plan considers reasonable for a covered service. You are not responsible for charges above this amount when using an in-network provider.
